Definition of a New Patient
A new patient is someone who has not been previously registered at either the Palo Alto Medical Foundation (PAMF) or Camino Medical Group (CMG).
Note: Current patients who need to update their contact information or insurance should call patient accounting at 650-812-3838 or fax any changes to 650-812-3739.

How to Register
To register as a new patient, first call 650-853-4868. Please have your insurance card ready. This process will take about 10 minutes. Then follow the steps listed below at your first appointment.
- Go directly to the registration check-in point before your first appointment. (For the Palo Alto Clinic, go to central registration in the main lobby on the first floor. For other locations, go to the check-in/arrival desk.)
- Bring your insurance card and photo identification (driver's license or I.D.).
- New uninsured patients need to make a $300 deposit, or a $500 deposit if their permanent address is not in the United States.
- You will be given forms which may include:
- Consent to Treat and Financial Agreement
- HIPAA (privacy form)
- PAMFOnline
- Medicare COB (Medicare patients only)
