To join the Encina Practice, you will need to review a Participation Agreement, schedule a get acquainted visit with an Encina physician, and submit an Application Form. The get acquainted visit will allow you to meet personally with an Encina physician and ask any questions about the practice to determine if Encina will meet your health care needs and expectations.
