Leaving Messages

When leaving a message, please be clear about your needs and leave multiple phone numbers, such as home and cell phone. If your call is urgent, indicate this to the receptionist so it can be handled appropriately.

Prescriptions

For new prescriptions, please give us your pharmacy phone number and any drug allergies.

To renew a prescription, have your pharmacist contact our centralized refill desk via fax at 408-730-4239. Please call two to three days ahead. If you need written prescriptions for mail order, please allow 72 hours for pick up.

Parking

The Orthopedic department is located on the first floor of the Mountain View Center. For your convenience, park on the lower level of the parking lot and you will enter the building on the first floor.
